This fixes some unwanted 'git status' output after 'make docs'.
* .gitignore: Add **/Makefile, **/makefile.
Remove Makefile, makefile.
Add doc/*/*.xxx rules for the usual kinds of documentation outputs
and temporaries. Add doc/misc/cc-mode.ss.
