mode is set or changed.
This fixes bug #15577 and bug #23407.

(hack-local-variables): Rename parameter `mode-only' to `handle-mode', make
its previous non-nil setting be t, and introduce the following action for a
non-nil non-t value: apply all settings apart from `mode'.

Reword the Emacs, Lisp intro, and Lisp reference manuals to eliminate
unnecessary quoting like this, and to use @dfn etc. instead when called
for (Bug#21472).
