Improve docstring of kmacro-set-format

* lisp/kmacro.el (kmacro-set-format): Improve docstring.
(kmacro-insert-counter, kmacro-display-counter)
(kmacro-set-counter, kmacro-add-counter): Add cross-references
to the info manual.
